The General Information Sheet is an annual SEC disclosure of current corporate, ownership, officer and beneficial-ownership information. The responsible team must use the current form, follow the applicable filing schedule and preserve proof of acceptance.

• Directors, trustees and officers
• Authorized, subscribed and paid-up capital

• Beneficial owners and reportable control

Preparation workflow

1. Download or generate the current official form.
2. Reconcile data with the stock and transfer book, minutes and prior GIS.
3. Investigate changes in ownership, officers, address and beneficial control.
4. Obtain review and signature from the authorized person.
5. File through the current SEC eFAST route or other officially prescribed channel.
6. Save the submitted file, receipt and acceptance or validation message.
7. Correct rejected or inaccurate filings promptly.

Deadline warning

The filing period depends on the corporation’s annual meeting and current SEC rules or extensions. Do not reuse a date from a prior year without checking the current official announcement.
